Mounjaro® and hiccups

People starting Mounjaro® sometimes notice bouts of hiccups, often alongside burping or a full, sour stomach. None of the official product information for tirzepatide lists hiccups, so this page is about the gap: what the labels do record close by, how a slower stomach could fit in, what else causes hiccups, and the point at which they stop being a nuisance and become a reason to call.

What the Mounjaro® labels record instead

The US prescribing information builds its main table from two placebo-controlled diabetes trials (SURPASS-1 and SURPASS-5, 953 adults, average exposure 36.6 weeks) and lists reactions seen in at least 5% of people on Mounjaro® and more often than on placebo [1]. Hiccups are absent there and in the section on postmarketing reports. A smaller list of stomach effects that were more frequent than on placebo is given in the text, and that is where the nearest relatives of hiccups sit [1].

The European product information places dyspepsia, bloating of the abdomen, eructation (burping), flatulence and gastro-oesophageal reflux disease in its “common” band, and delayed gastric emptying in the “uncommon” band [3]. The Canadian monograph lists the same group among reactions in at least 1% of patients [4]. Neither mentions hiccups.

We searched PubMed on 20 October 2026 for tirzepatide and hiccups and found no trial report, case report or pharmacovigilance study on tirzepatide. For a different GLP-1 medicine, semaglutide, one review of FDA safety reports counted 47 hiccup reports [8]; that says nothing directly about tirzepatide. For Mounjaro® this is an absence of evidence, not proof that the two are unrelated.

A neighbouring label that does mention hiccups

Hiccups are not unknown in this family of medicines. The Wegovy® (semaglutide) label lists hiccups among the stomach reactions that occurred more often than on placebo in its adult weight-reduction trials, together with dyspepsia, burping and reflux, and says these were reported mostly while the dose was being raised [5].

That is a different molecule and a different trial population, so its finding cannot be transferred to Mounjaro®. It does suggest that when hiccups appear on a GLP-1-based medicine, they tend to travel with the upper-stomach effects rather than on their own.

How a slower stomach could lead to hiccups

What the label supports: tirzepatide delays gastric emptying, and the US label adds that the delay is largest after the first dose and diminishes over time [1]. It also reports that most nausea, vomiting and diarrhea happened during dose escalation and then decreased [1]. On the standard schedule, the dose goes up in 2.5 mg steps after at least four weeks on the current one, so each step is a new adjustment period [1].

What is plausible but unmeasured for Mounjaro®: hiccups are spasms of the diaphragm set off when the nerves that control it are irritated, and eating too much or too fast, swallowing air and fizzy drinks are recognised triggers [6]. A stomach that stays full for longer, together with burping and reflux, could provide that irritation. No study has tested this for tirzepatide.

Timing may give a hint. After an injection, tirzepatide reaches its highest level in the blood somewhere between 8 and 72 hours, and it has a half-life of about five days [1]. If hiccups cluster in the day or two after your shot or after a dose step, that is worth writing down.

Causes that have nothing to do with Mounjaro®

Most short bouts of hiccups have no identifiable cause, according to MedlinePlus [7]. Cleveland Clinic lists triggers for brief episodes such as large or rushed meals, spicy or very hot or cold food, carbonated drinks, alcohol and smoking [6].

Hiccups that last for days have a different list: reflux disease or gastritis, conditions of the brain and nerves such as stroke or multiple sclerosis, lung conditions such as pneumonia, metabolic problems including kidney failure (uremia) and low calcium, some infections, and certain medicines, including dexamethasone, some benzodiazepines and some chemotherapy drugs [6]. Persistent hiccups can also follow surgery under general anaesthesia [6]. This longer list is why lasting hiccups are examined rather than waited out.

What may ease them

Cleveland Clinic says no hiccup cure has been proven, though brief holding of the breath, sipping or gargling cold water, or breathing into a paper bag are remedies people use, with anecdotal support [6]. MedlinePlus gives similar suggestions and warns never to try neck (carotid) massage on yourself [7].

For prevention, Cleveland Clinic suggests eating and drinking more slowly, smaller portions, fewer carbonated and alcoholic drinks, and avoiding very hot or cold foods [6]. Those steps also overlap with what helps when a slower stomach makes large meals uncomfortable. If burping or heartburn comes with the hiccups, the Mounjaro® heartburn page covers that side in more detail.

Hiccups lasting longer than two days may need prescription treatment chosen according to the cause [6]. Do not change your Mounjaro® dose or schedule to stop hiccups without talking to your prescriber.

Questions

Are hiccups a known side effect of Mounjaro?

No tirzepatide label lists hiccups. The US, EU and Canadian product information do list burping, indigestion and reflux, which can travel with them [1][3][4].

Why would hiccups start after a Mounjaro dose increase?

The US label reports that most stomach reactions occurred during dose escalation and eased over time [1]. Whether hiccups follow the same pattern has not been studied.

How long is too long for hiccups?

Cleveland Clinic calls hiccups lasting more than 48 hours persistent and advises calling a clinician at that point [6]. MedlinePlus says to call if they continue for more than a few days [7].

Does any GLP-1 medicine label mention hiccups?

Yes. The Wegovy® label lists hiccups among the stomach reactions seen more often than on placebo in its adult weight-loss trials [5]. Mounjaro® labels do not.

Do sipping cold water or holding my breath really work?

They are common home remedies with anecdotal support only; Cleveland Clinic notes that no proven hiccup cure exists and that most brief bouts stop on their own [6].

Can hiccups be a sign of something serious while on Mounjaro?

Brief bouts rarely are. Hiccups that last for days can be linked to reflux, nerve, lung, kidney or metabolic conditions, which is why they should be checked [6].
