Ozempic® and body aches

Aching muscles or stiff joints are easy to notice and hard to explain, especially when several things change at once: a new medicine, a new eating pattern, maybe more exercise. This page looks at what the Ozempic® product information says about muscle and joint pain, what large analyses of GLP-1 trials found, how muscle loss and fluid loss fit in, and which aches general medical sources treat as urgent.

Where aches stand in the Ozempic® documents

The three regulators’ documents were checked on 20 October 2026. None has a musculoskeletal row. The closest items are the injection site reactions, which are local, and the Canadian fatigue row, which also counts weakness (asthenia).

What trial analyses and cohorts report

A 2026 systematic review and meta-analysis combined 43 randomised trials of GLP-1-based medicines with 100,488 participants. It found no significant difference from control groups in reported gouty arthritis, rheumatoid arthritis, osteoarthritis, osteoporotic fractures, synovitis or spinal disc protrusion [4]. These were adverse events reported in the trials rather than conditions searched for systematically, which is a limitation the design cannot avoid.

For gout specifically, a 2024 network meta-analysis of 22 placebo-controlled outcome trials with 173,498 patients found GLP-1 medicines had no significant effect on gout risk, unlike SGLT2 inhibitors, which lowered it [5].

Observational data are less reassuring. A 2025 Nature Medicine study of US veterans with diabetes associated GLP-1 use with a higher risk of arthritic disorders than continued usual care [6]. Because people starting GLP-1 medicines may differ in many ways from those who do not, this association does not show that the medicine causes joint problems.

Why someone on Ozempic® might ache

From the label: low blood sugar, which section 5.5 links to combining Ozempic® with a sulfonylurea or insulin, can show up as weakness and shakiness [1]. Dehydration after vomiting or diarrhoea is a separate label warning [1]. MedlinePlus counts low potassium or calcium levels, an electrolyte imbalance, among possible causes of muscle pain, and advises drinking fluids around exercise [8].

Proposed and partly studied: losing muscle along with fat. A 2025 review reports that lean body mass can account for roughly 15% to 40% of the weight lost on GLP-1 medicines, and says evidence in older adults and people with kidney or liver disease is still limited [7]. Muscle loss is not the same as pain, but weaker muscles can make everyday effort feel harder.

General background: starting a new exercise routine alongside treatment, which many people do, is itself a common source of soreness. MedlinePlus lists overuse, too much too soon, and skipping a warm-up among the most common causes of muscle aches [8].

Other common causes

Most aches have ordinary explanations. MedlinePlus lists, among others [8][9]:

  • Injury, strains, sprains and overuse; tension and stress.
  • Medicines including statins for cholesterol and ACE inhibitors for blood pressure.
  • Viral infections such as flu, and other infections.
  • Arthritis, including osteoarthritis, rheumatoid arthritis and gout, as well as bursitis and tendinitis.
  • Fibromyalgia, lupus and polymyalgia rheumatica.
  • Less commonly, muscle breakdown (rhabdomyolysis).

What general sources suggest

For aches from overuse or minor injury, MedlinePlus describes resting the area, ice in the first days and heat afterwards, gentle stretching, massage, enough sleep and stress reduction [8]. It suggests warming up and cooling down around exercise, stretching through long periods at a desk, and building up activity gradually. For joint pain that is not from arthritis, it describes rest balanced with exercise, warm baths and stretching [9]. If you are considering a pain reliever, ask your pharmacist which one fits with Ozempic® and your other medicines.

On muscle loss, the 2025 review suggests that protein-focused eating and resistance training may help preserve muscle during GLP-1 treatment, tailored to the person [7]. How that applies to you is a question for your clinician or a dietitian.

Ozempic® context

Ozempic® is a weekly injection for adults with type 2 diabetes; the label starts at 0.25 mg and raises the dose after at least four weeks at each step [1]. The injection goes into the fatty tissue of the abdomen, thigh or upper arm, and local discomfort at the site is a listed but uncommon reaction [1][2]. Section 5.2 warns about pancreatitis, whose pain can spread from the stomach to the back [1]; back pain together with severe stomach pain should not be treated as an ordinary ache.

Questions

Does Ozempic® cause muscle or joint pain?

Neither is listed in the US, EU or Canadian product information for Ozempic® [1][2][3]. A 2026 meta-analysis of GLP-1 trials found no excess of reported musculoskeletal events [4].

Can Ozempic® trigger gout?

Gout is not in the Ozempic® labels. A 2024 network meta-analysis of outcome trials found GLP-1 medicines had no significant effect on gout risk [5].

Why do my muscles feel weak on Ozempic®?

Possible reasons include low blood sugar, especially with a sulfonylurea or insulin, dehydration, and loss of muscle along with fat [1][7]. Weakness with inability to move part of the body is an emergency [8].

I take a statin with Ozempic®. Could that be the cause?

Statins are among the medicines MedlinePlus lists as causes of muscle pain, and it advises contacting a provider if pain is linked to starting or changing a dose [8]. Do not stop either medicine on your own.

Is it normal to feel sore after starting exercise on Ozempic®?

Overuse and doing too much too soon are among the most common causes of muscle aches; MedlinePlus suggests warming up, building up gradually and drinking fluids around exercise [8].

How can I tell an injection site ache from a body ache?

Injection site reactions stay where you injected and are uncommon with Ozempic® [1][2]. Aches elsewhere, or in several places at once, have other causes worth noting separately.
