What the tablet’s documents list near the mouth
Halitosis appears in none of the three documents [1][2][3]. Several stomach reactions that can affect the breath do appear, with trial numbers in the US and Canada.
| Entry | Rybelsus® 7 mg / 14 mg | Placebo or status |
|---|---|---|
| Gastro-oesophageal reflux disease, Canadian Table 3 [3] | 1.7% / 2.2% | 0.3% |
| Burping (eructation), US section 6.1 and Canadian Table 3 [1][3] | 0.6% / 2.0% | 0% |
| Delayed gastric emptying, EU Table 1 [2] | Uncommon | Up to 1 in 100 |
| Altered taste (dysgeusia), US 6.2 and EU Table 1 [1][2] | Postmarketing (US); uncommon (EU) | No trial rate |

Three smells that need urgent attention
MedlinePlus describes several breath odours that point to illness rather than the mouth [7]. Three of them connect directly to warnings in the Rybelsus® documents.
- Fruity or sweet: a sign of diabetic ketoacidosis, a life-threatening complication of diabetes [7][10]. The EU leaflet lists a sweet smell on the breath, a sweet or metallic taste, heavy thirst, confusion and breathing difficulty among its signs, and warns that insulin doses reduced when Rybelsus® starts need closer glucose monitoring [2]. In US reports for semaglutide tablets, ketosis was among the strongest signals, though from only 4 reports [4].
- Like stool, after prolonged vomiting: MedlinePlus links this to bowel obstruction [7]. Ileus and intestinal obstruction are listed among reports received after approval for semaglutide tablets [1][3].
- Ammonia-like or urine-like: MedlinePlus links this to kidney failure, and lists breath odour and a metallic taste among symptoms of acute kidney failure [7][9]. The Rybelsus® label warns that dehydration from vomiting or diarrhoea can lead to acute kidney injury [1].
What dental researchers and reports describe
A 2026 narrative review of oral effects of semaglutide found dry mouth to be the most consistently reported, often secondary to nausea, eating less and slower stomach emptying, and listed halitosis among the additional effects alongside altered taste and dental caries [5]. A second 2026 review for dentists describes the same chain: semaglutide’s effects on stomach emptying, burping and reflux showing up in the mouth as dry mouth, bad breath, enamel erosion and taste changes [6]. Neither separated tablets from injections, and neither gives a rate for halitosis.
For semaglutide tablets specifically, a 2024 analysis of FDA reports found burping among the stronger stomach signals (35 reports) and regurgitation among the reported terms [4]. Our separate pages cover burping and dry mouth on Rybelsus® in more detail.
Why breath might change on the tablet
- Reflux and burping: Cleveland Clinic explains that reflux brings stomach acid back up, sometimes into the mouth, where it tastes sour and smells unpleasant [8]. Both happen more on Rybelsus® than on placebo [1][3].
- A slower stomach: semaglutide delays stomach emptying [1], so food stays longer before moving on.
- A drier mouth: Cleveland Clinic names dry mouth as a cause of bad breath because saliva rinses the mouth [8]; fluid loss from stomach side effects can add to it [1].
- Other diabetes medicines: MedlinePlus includes insulin among medicines that can cause breath odour [7], and many people take it alongside Rybelsus®.
Common causes unrelated to Rybelsus®
Cleveland Clinic calls poor oral hygiene the main cause of halitosis: bacteria left behind by missed brushing, flossing and cleanings produce bad smells and lead to cavities and gum disease [8]. Diabetes itself raises the risk of gum disease [8]. Other causes listed by Cleveland Clinic and MedlinePlus include tonsil stones, sinus, throat and lung infections, smoking, alcohol, coffee and strong foods such as garlic and onions, dentures, and large doses of vitamin supplements [7][8].
What general sources suggest
MedlinePlus recommends good dental hygiene, especially flossing, avoiding smoking, and notes that fresh parsley or a strong mint can cover temporary bad breath, while mouthwash does not fix the underlying problem [7]. Cleveland Clinic adds tongue scraping and says some rinses only mask odour while others act on the bacteria; a dentist can advise which [8]. If good hygiene does not help, it suggests seeing a dentist first and then a primary care provider if the mouth looks healthy [8].
The tablet routine adds one detail. Rybelsus® is swallowed on an empty stomach with a small amount of water, and the label asks for at least 30 minutes before any food or drink [1]. Mints, gum and flavoured drinks are best saved until that wait is over, so they do not interfere with how much semaglutide is absorbed.
