Rybelsus® and hair loss

Finding more hair in the brush is alarming, and with GLP-1 medicines it is a common worry. For Rybelsus®, the evidence is thinner than many people expect: the tablet trials did not report hair loss, and only the US label mentions it, among reports received after approval. This page explains what that means, what studies of semaglutide in other forms show, and which everyday causes of shedding are worth checking first.

What each Rybelsus® document says

The US label for Rybelsus®, which also covers Ozempic® tablets, lists alopecia under skin reactions reported during use after approval. It explains that such voluntary reports come from a population of unknown size, so neither a frequency nor a causal link can be established [1].

Why the tablet trials may say little

The Rybelsus® trials were in adults with type 2 diabetes, and weight change was modest. In the 26-week monotherapy trial, average weight fell by 2.3 kg on 7 mg and 3.7 kg on 14 mg, against 1.4 kg on placebo, from a starting weight of about 89 kg [1]. The Canadian table, which lists reactions down to 1%, does not include hair loss [3].

The clearest semaglutide data come from a different product and population. The Wegovy® label for weight management says hair loss on the injection was associated with weight reduction: 3.3% on 2.4 mg and 1% on placebo, more often in women than men [4]. That figure cannot be transferred to Rybelsus®, a diabetes medicine studied in a different population, but it shows the pattern regulators have seen with the molecule.

What studies across GLP-1 medicines report

A 2026 systematic review and meta-analysis of nine interventional studies, seven of them randomised, pooled 4,114 people using GLP-1 medicines and found hair loss about three times more likely than on placebo, with an overall event rate of 3.9% [5]. The abstract does not break results down by product, and the finding held when the analysis was limited to trials in people with overweight or obesity.

An analysis of US adverse-event reports from 2022 to 2025 found semaglutide reported with alopecia slightly more often than expected, while dulaglutide and tirzepatide were not [6]. Spontaneous reports cannot show how often hair loss occurs or prove the medicine caused it, and they do not separate tablets from injections.

Possible mechanisms, sorted by evidence

  • Supported by a label: in the Wegovy® trials, hair loss was linked to weight reduction rather than described as a direct drug effect [4].
  • Proposed but unmeasured for Rybelsus®: Cleveland Clinic lists rapid weight loss and diets low in protein among triggers of telogen effluvium [8]. Rybelsus® lowers appetite in some people (decreased appetite in 9.0% on 14 mg against 0.8% on placebo) [3], but no study has tied this to hair loss on the tablet.
  • A question for your prescriber: semaglutide tablets raised levothyroxine exposure by 33% in a drug interaction study, and the label suggests closer monitoring of such medicines [1]. Thyroid disease is itself a cause of hair loss [7], so if you take thyroid medicine, ask whether your levels should be checked.

Other common reasons hair thins

MedlinePlus lists causes that are far more frequent than any medicine [7]:

  • Heredity and age, the usual cause of gradual thinning in both men and women.
  • Telogen effluvium after high fever, severe infection, surgery, major illness or severe emotional stress, often noticed weeks to months later.
  • Crash diets, especially those without enough protein.
  • Other medicines, including beta-blockers, calcium channel blockers, some antidepressants and NSAIDs such as ibuprofen.
  • Thyroid disease, anaemia, hormone changes, scalp infections and tight hairstyles.

What general sources say about recovery

MedlinePlus explains that hair lost after an illness, fever or medicine use usually grows back once the cause has passed, and that hair pieces or a change of style can disguise thinning in the meantime [7]. Cleveland Clinic says acute telogen effluvium resolves in most cases, with regrowth typically visible three to six months after shedding starts [8].

Cleveland Clinic also advises avoiding extreme or restrictive diets and asking a healthcare provider before adding supplements [8]. Do not stop Rybelsus® on your own because of shedding; the European leaflet asks you to talk to your doctor before stopping, as blood sugar may rise [2].

Timing details that help your prescriber

Because shedding from telogen effluvium tends to start two to three months after a trigger [8], it can line up with a dose step or a period of weight loss rather than the day you started the tablet. Rybelsus® begins at 3 mg for 30 days, then 7 mg, with 14 mg possible from day 61 [1], so recording dates of dose changes, weight and any illness gives a much clearer picture than memory.

Questions

Is hair loss a side effect of Rybelsus®?

Only the US label mentions it, as alopecia reported after approval with no frequency. The European and Canadian documents do not list it [1][2][3].

Why does Wegovy® list hair loss but Rybelsus® does not?

Wegovy® is semaglutide for weight management at a higher weekly injected dose, and its label ties hair loss to weight reduction. The Rybelsus® diabetes trials did not report it [1][4].

When would hair loss start if it is related?

No Rybelsus® data address timing. Cleveland Clinic says shedding from telogen effluvium usually starts two to three months after a trigger [8].

Will my hair grow back?

MedlinePlus says hair lost after illness or medicine use usually grows back, and Cleveland Clinic says acute telogen effluvium resolves in most cases [7][8].

I take levothyroxine. Does that matter?

Semaglutide tablets raised levothyroxine exposure by 33% in a study, and the label suggests closer monitoring; thyroid disease can cause hair loss, so ask your prescriber about checking levels [1][7].

Should I take a supplement for hair loss on Rybelsus®?

Cleveland Clinic advises asking a healthcare provider before adding supplements [8]. Remember that other tablets need to wait 30 minutes after Rybelsus® [1].
