Wegovy® (injection) and frequent urination

Extra trips to the bathroom are a common online complaint from people starting Wegovy®, often alongside advice to drink more water. The product documents are quiet on urination itself, but they contain several details that bear on it: an infection figure in teenagers, high rates of constipation, kidney warnings about fluid loss and an animal finding. This page puts those pieces together and separates the changes that are expected from those that need attention.

Searching the Wegovy® documents

We searched the full US prescribing information, the EU product information and the Canadian monograph for frequent, excessive or night-time urination, including the medical terms pollakiuria, polyuria and nocturia. None appears as a reaction in adults, adolescents, the 7.2 mg dose or post-marketing lists. [1][2][3] The entries below are the urinary and fluid-related ones that do appear.

What research says about urine volume

Two lines of evidence are relevant. Mechanistic studies show that natural GLP-1 can make the kidneys release more sodium, but a review of controlled human experiments found this only when the body was already loaded with fluid, and those experiments lasted hours rather than months. [4] The Canadian monograph reports a short-lived increase in urine output in rats given semaglutide, among other effects seen in safety testing. [3]

Measurements in people point the other way. In 44 adults with a history of kidney stones who lost weight on GLP-1 based medicines over about a year, total 24-hour urine volume showed no significant change. [5] That group was small and had stones, so it may not represent everyone on Wegovy®, but it gives no sign that these medicines drive up urine output on their own.

Reasons you might still be going more often

  • Drinking more: the Medication Guide asks you to drink fluids to reduce the chance of dehydration, [1] and more fluid in means more urine out.
  • Constipation: it is one of the most common Wegovy® reactions and can last weeks. [1][2] NIDDK names constipation among the causes of bladder control problems, a link that has not been studied in Wegovy® users specifically. [8]
  • Infection: a strong, frequent urge, burning, cloudy urine and pressure low in the belly are typical bladder infection signs. [9] The teen trial figure above is the only urinary infection rate in the Wegovy® label. [1]
  • High blood sugar: Wegovy® is also used in adults with type 2 diabetes, and poorly controlled diabetes can cause frequent urination. [7]
  • Evening drinks, caffeine and alcohol, all named by MedlinePlus as common causes of waking to urinate. [7]

Other causes worth knowing

MedlinePlus separates two things that feel similar: needing to go often, and making a large total volume of urine. Its list of causes for night-time urination includes bladder or urinary infection, an enlarged prostate, pregnancy, chronic kidney disease, diabetes, heart failure, high blood calcium, water pills and swollen legs; it also links night waking to sleep apnoea. [7] For a large daily volume, common causes are diabetes, diabetes insipidus and drinking a lot of water. [10]

NIDDK lists overweight and obesity, diabetes, ageing, a chronic cough and nerve problems among causes of urinary leakage. [8] Several of these are common in people prescribed Wegovy®, and some may change as weight falls.

The urine change the label cares about

Section 5.5 of the Wegovy® label describes acute kidney injury reported after approval, in some cases needing dialysis, mostly in people who had nausea, vomiting or diarrhea leading to dehydration. Prescribers are asked to check kidney function in people with such reactions, especially when starting or increasing the dose. [1] In the adult trials, kidney injury occurred more often during dose escalation and in people who already had kidney impairment. [1]

For patients, this means that passing less urine, especially dark urine after a day of vomiting, deserves more concern than an extra bathroom trip. MedlinePlus lists decreased urination and swelling of the legs, ankles or feet among semaglutide side effects to report straight away. [6]

Practical steps from official sources

MedlinePlus suggests keeping a record of how much you drink, how often you urinate and roughly how much, and weighing yourself on the same scale at the same time each day. [7][10] It advises contacting a provider if night waking continues over several days or bothers you, and if large volumes persist without an explanation such as medicines or extra drinking. [7][10]

Cutting back sharply on fluids to avoid bathroom trips can work against the Wegovy® label, which asks patients to take precautions against fluid loss. [1] If evening trips are the problem, moving drinks earlier in the day is a common suggestion; check with your prescriber if you have kidney or heart disease. [7]

Questions

Does Wegovy® make you pee more?

It is not listed as a side effect in the US, EU or Canadian documents. [1][2][3] A small human study found no change in total daily urine volume with GLP-1 based medicines. [5]

Why do I pee more since starting Wegovy®?

Common reasons include drinking more, which the Medication Guide encourages, constipation pressing on bladder control, an infection or, with type 2 diabetes, high blood sugar. [1][7][8]

Can constipation on Wegovy® affect my bladder?

NIDDK lists constipation among the causes of bladder control problems, and constipation affected 24% of adults on Wegovy® vs 11% on placebo. [1][8] The link has not been studied in Wegovy® users directly.

My teenager on Wegovy® keeps going to the toilet. Should I worry?

Urinary tract infection was reported in 4% of adolescents on Wegovy® vs 2% on placebo. [1] Burning, cloudy urine or fever are reasons to see a clinician. [9]

Is it a problem if I am peeing less?

It can be. The label links fluid loss from stomach side effects to kidney injury, and MedlinePlus lists decreased urination as a semaglutide side effect needing a doctor straight away. [1][6]

Should I drink less in the evening?

MedlinePlus suggests avoiding a lot of fluid before bed for night-time trips. [7] Keep total fluids up during the day, as the label asks, and check with your prescriber if you have kidney or heart disease. [1]
