Three documents, no insomnia row
Each set of official product information counts side effects in its own way, so it is worth checking all three before concluding anything. The US table includes reactions reported by at least 2% of people on Wegovy® and more often than on placebo; the Canadian table goes down to 1%; the EU table sorts reactions into frequency bands. [1][2][3] Insomnia does not appear in any of them, nor in the US post-marketing section or the Medication Guide. [1]
| Source | Who was counted | Insomnia |
|---|---|---|
| US label, Table 3 [1] | Adults, 2.4 mg pool, 68 weeks (≥2% and above placebo) | Not listed |
| US label, Table 4 [1] | Adults, 7.2 mg trials, 72 weeks | Not listed |
| US label, Table 5 [1] | Adolescents 12 and older, 68 weeks (≥3%) | Not listed |
| EU SmPC section 4.8 [2] | Adults, STEP 1–4 pool, plus post-marketing reports | Not listed |
| Canadian monograph, Table 4 and section 8.3 [3] | Adults, STEP 1–3 (≥1%), plus less common reactions | Not listed |
What research has measured
A retrospective cohort built from US electronic health records compared people with type 2 diabetes who were prescribed semaglutide with matched groups on sitagliptin, empagliflozin or glipizide. Insomnia was one of 22 neurological and psychiatric outcomes tracked over 12 months, and semaglutide was not associated with a higher risk of any of them. [5] The study was observational and exploratory, involved diabetes doses rather than the Wegovy® weight-management dose, and cannot rule out every difference between the groups.
For contrast, the liraglutide label for weight management does list insomnia, at 2.4% versus 1.7% on placebo. [4] Liraglutide is a different molecule taken daily, so that figure cannot be transferred to Wegovy®.
How a weekly injection might still cost you sleep
- In the label: digestive reactions were most frequent during dose escalation, and reflux affected 5% of adults on Wegovy® versus 3% on placebo. [1] Reflux and nausea are uncomfortable when lying down, although the label does not connect them to sleep.
- In the label: Wegovy® raised resting heart rate by an average of 1 to 4 beats per minute, and the label asks people to report palpitations or a racing heartbeat at rest. [1] A pounding heart at bedtime is worth mentioning for that reason alone.
- In the label, for type 2 diabetes: low blood sugar is more likely when Wegovy® is combined with insulin or a sulfonylurea, and its signs include sweating, shakiness, anxiety and a fast heartbeat. [1]
- General: MedlinePlus lists stress, depression, caffeine, alcohol, tobacco, shift work, travel across time zones, other medical conditions and other medicines as common drivers of insomnia. [6] Any of these may change while a new treatment starts.
Other things that keep people awake
MedlinePlus separates short-term insomnia, often triggered by stress or an upsetting event and lasting days or weeks, from chronic insomnia lasting a month or more, which is usually a symptom of another problem such as a medical condition, a medicine or another sleep disorder. [6] Depression and emotional distress raise the risk. [6]
Sleep breathing problems deserve a mention too. Obstructive sleep apnea was common among people entering the STEP weight trials, recorded in 11.7% to 15.1% of participants at the start of STEP 1 to 4. [3] Wegovy® is not approved to treat it, and snoring, gasping or daytime sleepiness are worth raising with a clinician on their own merits.
Steps general guidance describes
MedlinePlus describes habit changes that often help short-term insomnia: getting up and going to bed at similar times, keeping the bed for sleep, avoiding caffeine and alcohol in the evening, avoiding heavy meals close to bedtime, keeping the room quiet and dark, and getting up to do something calm if sleep does not come. [7] It also suggests writing down worries before bed and finding a relaxation routine. [7] For insomnia that lasts, it notes that a form of counselling called cognitive behavioural therapy can help. [6]
Two Wegovy®-specific points sit alongside this. The label lets the injection be given at any time of day and allows the time of day to change without a dose adjustment. [1] And the Medication Guide asks you to tell your provider about every medicine and supplement you take, including over-the-counter products, because Wegovy® slows stomach emptying and can affect how some medicines are absorbed. [1] That applies to sleep aids too, so check with your pharmacist or prescriber first.
