Wegovy® (injection) and stomach pain

Stomach pain is common on Wegovy® injection, and most of it belongs to the familiar digestive picture. A few kinds of pain, though, are singled out in the product information as possible signs of pancreatitis, gallbladder disease or appendicitis. This page covers both: the US, EU and Canadian figures, what trial analyses add, how the label’s warning pains are described, the many other causes of belly pain, what general guidance says for mild pain, and how fast to act on each warning sign.

A broad term, counted in five places

“Abdominal pain” in the Wegovy® tables is a grouped term. A footnote explains that it combines abdominal pain, upper and lower abdominal pain, gastrointestinal pain, abdominal tenderness, abdominal discomfort and epigastric discomfort, meaning discomfort just below the breastbone. [1] So the row captures anything from a dull ache to sharper pain, wherever in the belly it was felt. The EU and Canadian documents use comparable grouped terms. [2][3]

Two adult pools, two different gaps

In the original US adult pool, people on 2.4 mg reported abdominal pain twice as often as those on placebo. In the newer 7.2 mg trials the 2.4 mg arm showed a smaller gap (9% against 7%), while 7.2 mg came in at 12%. [1] The pools enrolled different people over different periods, which is why the label keeps them in separate tables.

The EU product information describes most abdominal pain events as mild to moderate and of short duration, and marks abdominal pain as mainly seen during dose escalation. [2] A pooled analysis of STEP 1–3 found 98.1% of digestive events on semaglutide 2.4 mg were mild to moderate and clustered during or shortly after the dose increases. [4]

Teenagers reported more of it

In the adolescent trial, 15% on Wegovy® and 6% on placebo reported abdominal pain, a gap of nine percentage points. [1] The same trial recorded gallstones in 3.8% of teenagers on the injection and none on placebo, and section 5.3 says gallstones and gallbladder inflammation were more frequent in teenagers on the injection than in adults. [1] For a young person with new upper belly pain, that difference is worth knowing.

Pains the label treats as warnings

  • Pancreatitis (US section 5.2): belly pain that is severe or keeps going, at times spreading through to the back, with or without nausea or vomiting. In adult weight-reduction trials, acute pancreatitis was confirmed in 4 people on Wegovy® and 1 on placebo. If it is suspected, the medicine is stopped. [1]
  • Gallbladder disease (section 5.3): gallstones were reported in 1.6% of adults on the injection and 0.7% on placebo, and gallbladder inflammation in 0.6% versus 0.2%. The Medication Guide lists upper stomach pain, yellow skin or eyes, fever and clay-coloured stools, and notes some gallbladder problems need surgery. [1]
  • Appendicitis (section 6.1): including perforated appendicitis, in 10 adults on Wegovy® (0.5%) and 2 on placebo (0.2%) in the weight-reduction trials. [1] The Canadian leaflet asks people with its symptoms to stop the medicine and get immediate medical help. [3]
  • Blockage (section 6.2): ileus and intestinal obstruction are listed among post-approval reports, without a rate. [1]

Why ordinary stomach pain may happen

The label supports a general link: digestive reactions, including abdominal pain, occur more often on Wegovy® than on placebo and concentrate around dose increases, and semaglutide slows stomach emptying. [1] It does not explain abdominal pain on its own. Other listed digestive effects can bring pain with them: in the adult pool, indigestion was reported by 9% versus 3%, bloating by 7% versus 5%, and gas by 6% versus 4%. [1]

What research adds: a meta-analysis of 55 placebo-controlled trials with more than 106,000 participants found GLP-1 medicines increased the risk of gallstones (about 2 more cases per 1,000 people) and probably reflux disease, but probably had little or no effect on pancreatitis, obstruction and most other serious digestive events. [5] That helps put the label’s warnings in proportion without dismissing them.

Other causes of belly pain

MedlinePlus notes that how bad belly pain feels does not always match how serious the cause is: a stomach virus can hurt a lot, while early appendicitis may hurt only a little. [6] Pain spread across most of the belly is more typical of a virus, indigestion or gas; pain in one spot points more towards an organ such as the appendix or gallbladder; and pain that comes in waves is often caused by kidney stones or gallstones. [6]

Less serious causes include constipation, irritable bowel syndrome, food intolerance, food poisoning and stomach flu. Others include ulcers, reflux, diverticulitis, kidney stones, urinary infections, hernia, muscle strain, inflammatory bowel disease and, in women, menstrual cramps, endometriosis, ovarian cysts or an ectopic pregnancy. [6]

Home care for mild pain

For mild belly pain, MedlinePlus suggests sipping water or other clear fluids, avoiding solid food for the first few hours, and returning to small amounts of mild food after vomiting. It notes that people with diabetes need to check their blood sugar often when not eating normally, and says not to take any medicine without talking to a provider. [6] Novo Nordisk’s patient page lists stomach pain and upset stomach among common side effects to raise with your healthcare professional. [7]

The Wegovy® Medication Guide asks you to report stomach trouble that is severe or persistent, and to list every medicine you take, including over-the-counter antacids. [1] Severe digestive reactions were reported in 4.1% of adults on the injection versus 0.9% on placebo. [1]

Questions

How common is stomach pain on Wegovy®?

In the adult 2.4 mg trials, 20% of people on the injection reported abdominal pain against 10% on placebo. In the 7.2 mg trials it was 12% on 7.2 mg. [1]

When is stomach pain on Wegovy® a warning sign?

The label singles out persistent or severe pain, sometimes reaching the back, as a possible sign of pancreatitis, and upper stomach pain with fever, yellowing or pale stools as possible gallbladder trouble. [1]

Did the Wegovy® trials report appendicitis?

Yes. In the adult weight-reduction trials, appendicitis occurred in 10 people on Wegovy® (0.5%) and 2 on placebo (0.2%). [1]

Does Wegovy® stomach pain go away?

The EU product information describes most abdominal pain events as mild to moderate and of short duration, mainly during dose escalation. [2] Pain that is severe, persistent or different from usual needs a call. [1]

My teenager on Wegovy® has upper belly pain. Is that different?

Gallstones were more frequent in teenagers on the injection than in adults (3.8% versus none on placebo in the adolescent trial), so upper belly pain with fever or yellowing should be reported promptly. [1]

Can I take an antacid for stomach pain on Wegovy®?

MedlinePlus says antacids may help pain high in the belly after meals, but not to take any medicine without talking to a provider. [6] The Wegovy® Medication Guide asks you to report over-the-counter medicines. [1]
