Status: not an approved medicine
On the day this page was updated, FDA’s label database (openFDA) returned no label for retatrutide. Products sold under the name outside clinical trials are not verified to be the trial drug, and the data below cannot be assumed to apply to them [5].
Most frequent events reported in TRIUMPH-2
TRIUMPH-2 was an 80-week, placebo-controlled phase 3 trial in 1,152 adults with obesity and type 2 diabetes. Its published abstract reports these rates [1]:
| Adverse event | Placebo % | Retatrutide 4 mg % | 9 mg % | 12 mg % |
|---|---|---|---|---|
| Diarrhoea | 13 | 27 | 34 | 34 |
| Nausea | 8 | 14 | 21 | 28 |
| Hypotension (low blood pressure) | <1 | 1 | 5 | 6 |
| Dysesthesia (abnormal skin sensations) | 1 | 4 | 6 | 7 |
Dose columns are trial arms, not a recommendation. Rates from one trial cannot be compared directly with other trials [1].
Other published trials
- Phase 2 obesity trial (NEJM 2023, 338 adults): the most common events were gastrointestinal, dose-related, mostly mild to moderate, and partly reduced with a lower starting dose. Dose-dependent heart rate increases peaked at 24 weeks and declined afterwards [3].
- TRIUMPH-1 (NEJM 2026, 2,339 adults with obesity without diabetes, phase 3): the most common adverse events were gastrointestinal [2].
- Phase 3 trial in type 2 diabetes (Lancet 2026, 537 adults, 40 weeks): the most frequent events were generally mild to moderate gastrointestinal events that subsided over time; discontinuation due to adverse events was 2–5% with retatrutide and 0% with placebo; no severe hypoglycemia was reported [4].
Do retatrutide side effects go away?
The published trials describe the digestive events as mostly mild to moderate. In the phase 2 obesity trial they were dose-related and partly reduced with a lower starting dose [3]; in the phase 3 type 2 diabetes trial they subsided over time [4].
There is no label yet, so there is no official statement on how long they last.
Does retatrutide affect heart rate or blood pressure?
In the phase 2 trial, heart rate rose with dose, peaked at 24 weeks and declined afterwards [3]. In TRIUMPH-2, hypotension (low blood pressure) was reported by 1–6% on retatrutide vs under 1% on placebo [1].
