Wegovy® (injection) and diarrhea

Diarrhea is the second most common reaction in the adult Wegovy® injection table, reported almost twice as often on the medicine as on placebo. This page reads the US label closely and sets it beside the EU and Canadian product information: which trials recorded it, how long bouts lasted, how often it led people to stop, why fluids and kidneys come into it, what else can cause loose stools, what general guidance says about home care, and when diarrhea needs a doctor.

Diarrhea in the official documents

In the US label, Table 3 pools three placebo-controlled weight-reduction trials in adults on 2.4 mg lasting up to 68 weeks (2,116 people on the medicine, 1,261 on placebo), and Table 5 comes from a 68-week trial in 201 adolescents aged 12 and older. [1] The EU product information pools STEP 1–4, and the Canadian monograph pools STEP 1–3. [2][3]

The row missing from the 7.2 mg table

Table 4 summarises two 72-week trials that compared 7.2 mg, 2.4 mg and placebo. A reaction appears there only if it was reported by at least 2% of the 7.2 mg group and more often than in both the 2.4 mg and placebo groups. Diarrhea does not appear, so the label gives no diarrhea percentage for 7.2 mg. [1]

That absence is easy to over-read. It does not mean nobody on 7.2 mg had diarrhea, and it does not show a lower rate than 2.4 mg. It only means diarrhea did not pass that particular test. For these trials the label reports digestive reactions as a whole through discontinuations: 3% on 7.2 mg, 3% on 2.4 mg and 0.3% on placebo stopped because of them. [1]

Timing, duration and stopping

The US, EU and Canadian documents all tie digestive reactions mainly to the weeks when the dose is being raised. [1][2][3] Section 2.2 of the US label builds the injection up from 0.25 mg over 16 weeks in four-week steps, and allows a step to be delayed by four weeks when a dose is poorly tolerated. [1]

The EU product information and Canadian monograph describe most digestive events as mild to moderate and short, with a median diarrhea episode of 3 days. [2][3] A pooled analysis of STEP 1–3 found 98.1% of digestive events were mild to moderate and most occurred during or shortly after dose escalation. [4] A US safety-database study adds a timing pattern from voluntary reports: digestive problems tended to start within the first weeks and their reporting fell over time, although such reports cannot give a rate or prove cause. [5]

Diarrhea was the third most common reason adults on 2.4 mg stopped Wegovy® for good, at 0.7% against 0.1% on placebo, after nausea (1.8%) and vomiting (1.2%). [1]

The teenage figure

Among adolescents, 22% on Wegovy® and 19% on placebo reported diarrhea. That three-point gap is much narrower than in adults, and a placebo rate near one in five is a reminder that loose stools are common in young people for many reasons. In the same trial, nausea (42% versus 18%) and vomiting (36% versus 10%) differed far more. [1] Overall, 62% of teenagers on the injection and 42% on placebo had at least one digestive reaction. [1]

Why it may happen, and why fluids matter

  • What the label supports: digestive reactions cluster around dose increases, and the gradual climb is meant to lower that risk. Semaglutide also slows stomach emptying. [1] The label does not explain diarrhea specifically.
  • The fluid chain the label does describe: diarrhea, nausea and vomiting can cause dehydration, and dehydration can lead to kidney problems. Section 5.5 asks prescribers to monitor kidney function in people with reactions that could drain fluids, especially when starting or raising the dose. [1] In the adult weight trials, acute kidney injury occurred in 7 people on Wegovy® and 4 on placebo, some alongside digestive reactions or dehydration. [1]
  • Who may be more affected: the EU document says people with moderate kidney impairment may have more digestive effects on semaglutide. [2]
  • General background: NIH lists medicine side effects, together with viral infections and food poisoning, as the most common causes of short-term diarrhea. [6]

Other causes of loose stools

Not every bout on Wegovy® comes from Wegovy®. NIH names stomach bugs such as norovirus, food poisoning and travellers’ diarrhea, as well as antibiotics, magnesium-containing antacids and liquid medicines or sugar-free sweets containing sugar alcohols such as sorbitol and xylitol. [6] Diarrhea that lasts can also come from lactose or fructose intolerance, coeliac disease, inflammatory bowel disease, irritable bowel syndrome, diabetes-related nerve problems or previous gallbladder or bowel surgery. [6] Fever, chills or blood in the stool point more towards infection and need a doctor. [6]

Home care that general guidance describes

NIH says most short-term diarrhea can be managed at home by replacing lost fluids and electrolytes with water, broths, sports drinks or oral rehydration solutions. It adds that older adults and people with diabetes, kidney disease or a weakened immune system should check with their doctor first about which solution and how much. [7] MedlinePlus lists thirst, dry mouth, passing little urine, darker urine, headache and muscle cramps as early signs of dehydration. [8]

NIH notes that over-the-counter medicines such as loperamide can treat many cases in adults, but not with bloody stools or fever, and that you should see a doctor if diarrhea worsens or lasts more than 2 days while taking one. [7] The Wegovy® label does not address anti-diarrhoea medicines; its Medication Guide asks you to tell your provider about every over-the-counter medicine and supplement you take. [1] Ask your pharmacist or prescriber before using one, and remember that Novo Nordisk’s patient page lists diarrhea among the common side effects to talk over with your healthcare professional. [9]

Questions

Does Wegovy® 7.2 mg cause more diarrhea than 2.4 mg?

The label does not say. Its 7.2 mg table only lists reactions that were more frequent on 7.2 mg than on both 2.4 mg and placebo, and diarrhea is not among them; no separate rate is given. [1]

How long does diarrhea from Wegovy® last?

In STEP 1–4, the median episode lasted 3 days in people treated with semaglutide. [2] NIH advises adults to see a doctor if diarrhea lasts more than 2 days. [6]

Does diarrhea mean Wegovy® is working?

A pooled analysis of STEP 1–3 found average weight loss was similar in people with and without digestive side effects; less than one percentage point of the extra loss was linked to them. [4]

Can I take loperamide (Imodium) for Wegovy® diarrhea?

The Wegovy® label does not cover anti-diarrhoea medicines. NIH says loperamide can treat many cases of short-term diarrhea in adults, but not with bloody stools or fever. [7] Ask your pharmacist or prescriber first and mention Wegovy®. [1]

How common is diarrhea in teenagers on Wegovy®?

In the adolescent trial, 22% on the injection and 19% on placebo reported diarrhea, a much smaller difference than in adults. [1]

Why does diarrhea on Wegovy® matter for the kidneys?

Lasting diarrhea can cause dehydration. The label links dehydration from digestive reactions to reports of acute kidney injury and asks that ongoing diarrhea be reported promptly. [1] The EU document adds that people with moderate kidney impairment may have more digestive effects. [2]
