Semaglutide vs tirzepatide: how the labels describe them

Semaglutide and tirzepatide are the active ingredients behind five US brands. This page sets out how their labels describe each one, without ranking them.

How each label describes the ingredient

  • Semaglutide: a GLP-1 receptor agonist. Injected weekly (Ozempic®, Wegovy®) or taken by mouth daily (Rybelsus®, Ozempic® tablets, Wegovy® tablets) [1][2][3].
  • Tirzepatide: a GIP receptor and GLP-1 receptor agonist, injected weekly (Mounjaro®, Zepbound®) [4][5].

What the labels have in common

All of these labels carry a boxed warning about thyroid C-cell tumors, list digestive effects such as nausea, diarrhea, vomiting and constipation among the most common side effects, and start at a low dose that is raised in steps.

Timing facts from the labels

Each label’s own numbers from section 12.3 and section 2. These are timing facts, not a switching schedule, and they do not turn one dose into another.

Using them together

The Wegovy® label advises against use with any other GLP-1 receptor agonist [3]; the Zepbound® label advises against combining it with other tirzepatide products or any GLP-1 receptor agonist [5].

Moving between them, in either direction

Neither the Wegovy® (injection) label nor the Zepbound® label sets out a schedule for moving from one to the other [3][5]. The timing facts above describe each medicine on its own; they are not a plan for a change. If a switch comes up, in whichever direction, the gap, the first dose of the new medicine and its steps are set by your prescriber.

Questions

Is tirzepatide a GLP-1?

Its labels describe it as a GIP receptor and GLP-1 receptor agonist: it acts on both. Semaglutide is described as a GLP-1 receptor agonist [3][5].

Can I compare semaglutide and tirzepatide doses in milligrams?

The labels do not do that. Each product lists its own dose steps, and a milligram of one ingredient is not equivalent to a milligram of the other.
